To get it to work in DOSBOX you need a section as follows in your DOSBOX.CONF:

You can pick any base, IRQ, DMA, as long as it does not conflict with Sound Blaster settings, and for the directory choose the one where you placed the patches (assuming it's mounted).

With these configuration I got MK2 to work nicely with the GUS. I needed to set the volume to 115, lower than the default 127, otherwise the music is too loud and drowns the sound effects. This is done by running ULTRAMID -m115 (instead of just ULTRAMID). You may find that you need slightly different volume levels to achieve good sfx/music balance.
